The attachments below offer 2 options. 

• Print out the document, complete the paperwork and bring it with you to your consultation appointment with Dr. Schaffer.
• Download the fillable, PDF onto your computer FIRST. After completing the forms, save them to your computer and attach them in an email to office@flpain.com. IF YOU DO NOT DOWNLOAD A COPY FIRST, the information is lost when you try to attach to the email.
